Knox County has one of the highest rates in the state for diabetes-related deaths located in the “Diabetes Belt” by the Centers of Disease Control. Due to limited resources, available in the county, Community members are often requesting information and support from the local Extension Service. Local health care providers refer patients to the Family Consumer Sciences Agent when they are needing help managing their diabetes. Diabetes is a prevalent disease in the state of Kentucky, as well as in Knox County. A review of statistics from Countyhealthrankings.org and Kentuckyhealthfacts.org indicate that 19% of the Knox County population suffer from Diabetes, 43% of the population are obese and 8% of adults are without health insurance.
To reduce chronic disease risk and debilitation, “Taking Ownership of Your Diabetes” series was offered by the Knox County Extension Service Family and Consumer Sciences. The curriculum was taught by Family and Consumer Sciences Agent weekly over a series of 12 lessons. The curriculum includes strategies such as goal setting and problem solving to empower individuals to manage their diabetes.
They were given a formal evaluation that consisted of a pre-and a post test. Participants reported that because of the program:
•67 % took part in physical activity 30 minutes or more on five or more days a week
•100% were seen by a health professional to help manage their diabetes
•67% checked their blood glucose 1 or more times a day and 100% have seen an improvement
•100% had their AIC checked two or more times this year
•100% implemented at least 3 healthy eating practices to help address their diabetes
•There were a total of 24 behavior changes made in relation to eating habits.
•100% set one of more goals for themselves
•100% accomplished at least one goal
•100% have used a diabetes meal plan and comply with the regimen prescribed by their doctor
•100% are better problem solvers in their daily lives as they deal with their diabetes
