Situation:

County Extension Council,Family Consumer Sciences Advisory Council and Health Action Team members were among those who contributed to the discussion of identifying the educational needs and resources of most need to our individuals and families in Lawrence County. Program needs included the need to continue diabetes education and chronic disease prevention. The prevalence of adult diabetes in Lawrence County is at an all time high, 21% according to the CDC, KY Department for Public Health. (BRFSS) Lawrence County poverty rate for all ages is an average of 25.9%. (KY Health Facts) 199 grandparents lived with and were responsible for their own grandchildren; 28.7% of families with related children live below the poverty level; 24.7% of the senior population (65 years or older) live below poverty; and 65.9% of students are eligible for free lunches (American Community Survey).

Long-Term Outcomes:

* Community social, environmental, and policy systems will reinforce healthy lifestyle behaviors and nurturing safe environments for all ages.

* Individuals will reduce their risk for diabetes and other chronic diseases by making beneficial lifestyle changes which will lower the percentage of prevalence for diabetes and other chronic diseases.

*Youth and families will become more health conscious by incorporating physical exercise and healthy food choices into their daily regimen.

*Youth will reduce the likelihood of experimenting with drugs, alcohol and substances by becoming more educated and aware of the long-term effects.

Intermediate Outcomes:

*Partners will design opportunities to support individuals and families toward living a healthier lifestyle in a nurturing safe environment.

*Individuals and families will adopt one or more behavioral changes by participating in educational programs focusing on diabetes, chronic disease prevention and/or the related topics of health and well-being for all ages.

*Community partners and 4-H will work together to develop programming that caters toward a more healthy and active lifestyle.

*The school system will continue to allow 4-H and related community partners to educate our youth with drug, alcohol and substance abuse programming.

Initial Outcomes:

*Community partners will recognize the need for community social, environmental and policy systems to address efforts toward improving the health and well-being of individuals and families.

*Individuals and families will gain knowledge and develop skills to live a healthy lifestyle in a nurturing safe environment.

*Youth and families will acquire knowledge to incorporate healthier eating and physical exercise into their daily lives.

*Our schools and community will recognize and acknowledge the need to educate our youth on the implications of using drugs, alcohol, and other substances.

Evaluation:

Initial Outcome: Individuals will gain knowledge and develop skills to manage or prevent chronic illnesses.
Indicator: Number of individuals reporting positive knowledge change and skill development related to the management and/or prevention of chronic illnesses (including diabetes, addiction, etc.)

Major Program: Active Living and Health Promotions General

Nearly 50% of adults in the United States do not drink enough water. The human body consists of about 60% water, and if you don’t drink enough water, dehydration can have a variety of effects on the body. Studies show that dehydration can even impair aspects of brain function.Upon observation, youth in Lawrence County don’t drink nearly enough water. Children are often seen carrying around sugary drinks such as soda and juices. Children take behaviors and habits learned in childhood
