Situation:
People are living longer and are expected to continue to live longer throughout the lives. The goal of Embracing Life As We Age is to develop programs and work with others initiatives to help individuals, families mange the challenges and discover the positive aspects of life transitions and growing older.
Long-Term Outcomes:
Encourage people to practice a healthy lto promote healthy aging.
Intermediate Outcomes:
Help people to live a longer, healthy lifestyle and live independently.
Initial Outcomes:
A change in the awareness, knowledge, and aspirations related to healthy aging, independent aging and quality of life.

Initial Outcome: Healthier Lifestyle
