Situation:

The obesity epidemic threatens the quality and years of life of Pulaski County people.  Obese individuals are at increased risk for many chronic health conditions, including type 2 diabetes, heart disease, stroke, and some types of cancers.  The obesity rate in Kentucky increased 90 percent over the last 15 years.  The goal of the Making Healthy Lifestyle Choices is to reverse these trends by working with various organizations, agencies and groups to promote the health and wellness in all Pulaski County.

Long-Term Outcomes:

To improve the eating habits of people and  improve their lifestyle.

Intermediate Outcomes:

To develop nutrition messages and programs that will allow individuals to make informed decisions regarding healthy lifestyle choices.

Initial Outcomes:

To manage healthy food choices throughout the lifecycle through making healthy lifestyle choices.

Evaluation:

Initial Outcome:Change in awareness, knowledge, opinions, skills and attitudes needed to make informed choices regarding their lifestyle.

Indicator: Change in lifestyle in eating and physical activities.

Method:  Education and activities

Timeline:  July, 2020 through June 2021

Intermediate Outcome:  Practice healthy lifestyle

Indicator:  More involved in physical activities and change of food eating

Method:  Education classes

Timeline:  July, 2020-June 2021


Long-term Outcome:

Indicator:  A decrease in the number of obese and overweight people in the county.  A decrease in chronic disease.

Method:  Weigh in and physical activity.
